I've recenty significantly revamped an message-based form in Outlook
2003 (with SP3).

The form has been published to the organisational forms folder for all
to access. originally it was published with the "send definition with
form" box ticked. Result - everyone could produce, send and read the
form but when trying to reply everyone got the message "Unable to open
this form".

I then republished with the send definition box unticked. Now the
form is completed and send correctly but comes up as a standard mail
form at the other end with all of the data missing.

There is some small amount of code in the form - action buttons set to
work as hyperlinked.

I've gone through the form to make sure all of the fields are in the
item rather than the folder. Any other ideas?
