It's a very rare problem, so rare that I don't remember the exact cause or solution. Sorry. You could always try a completely new mail profile; that's one of the standard troubleshooting steps. I wouldn't expect reinstalling to do anything that Detect and Repair hasn't already done.
Your custom forms are stored in the .pst file so they should be fine. You did make backup .oft file copies of them, as well, didn't you?

already tried both those based on other responses I've seen from you.
Neither of those had any effect.
Any other thoughts? Does the Standard Forms Library missing elements
seem related? Maybe I should re-install the application. If I do re-
install, is there a way to wipe the registry so there's no residual
problem? Also, is there a way to preserve my existing custom forms?
