Before i start, can i just say that i am not normally this lazy and will generally research things myself. However time at work has not allowed me to do so. I have never used outlook forms before, but wish to make a change control request form for my new application that is being released in the next few weeks. Can anyone advise of a good starting point. I have played about with some text boxes on the form, but when i send it the email looks like a blank email. Thanks Nathan.
